Protected error reporting for physicians
- Kind
- Policy and health systems
Prompt
In commercial aviation, pilots can report their own mistakes and near misses to a system that protects them from being punished for what they report. Should physicians who report their own mistakes be given the same protection, even if it means some careless physicians are never disciplined?

How to approach this question
This panel interview question tests whether you understand the tradeoff between learning from mistakes and holding individuals accountable, a central idea in patient safety. Explain why protection might matter, since people tend to hide errors they'll be punished for, and hidden errors repeat. Then take the cost seriously, because some careless physicians would escape discipline and patients they harmed may feel denied justice. Consider where a line could sit, such as protecting honest reports while keeping discipline for recklessness, repeated harm or a cover-up. Think about what patients and families are owed when a mistake affects them. Finish with a clear position and the conditions that would make it work.
